The following forms are designed to help us work together more easily. All new clients need to complete the new client intake form and the general liability release form. Anyone under the age of 18 is considered a minor and the minor liability release form must be completed by a parent prior to any received treatments.
If you are a new client, please complete this form and bring it along to your first session.
General Liability Release Form
This is a standard release of liability for our massage therapy work together.
This form allows a parent or legal guardian to give permission to perform massage on a minor client.
